ENROLLMENT INSTRUCTIONS &
FOR NEW APPLICANTS
Welcome to the Open Enrollment Process for the Marlington
The Open Enrollment Period for the 2019-2020 school year is April
15, 2019 through May 15, 2019. All interested
parent(s)/guardian(s) must complete an Application form and submit the
documentation outlined below. Only a parent or guardian who has legal
custody may complete an Open Enrollment Application. You will need to provide court documentation
to show legal custody if there has been a divorce or separation.
Applications forms can be obtained ONLY at the Administration Offices or by clicking here beginning on April 15th and ending on May 15th. After completion, the application must be
returned to the Administration Offices, which is located at the middle school
at 10325 Moulin Avenue, NE, Alliance, Ohio 44601, Monday through Friday, from
8:00 a.m. to 3:00 p.m. ENTRY THROUGH DOOR 15 ONLY.

enrollment questions and can be reached at 330-821-4016 or by e-mail at email@example.com. All applications will be dated and time-stamped upon receipt. You will be notified no later than June 15, 2019 of your acceptance or denial of open enrollment.
IN ADDITION TO THE
COMPLETED APPLICATION, the following documents will be required at time of application for open enrollment:
Ö Child’s Birth Certificate
Social Security Card
Ö Two (2) Proofs of Residency (ie: utility bills, parent/guardian driver’s license; mortgage, rental or lease agreement voter registration card)
Ö Immunization Records
Ö Proof of Legal Custody (if applicable)
Ö Copy of Current IEP and ETR (if applicable)
If your student is accepted for open enrollment, all information collected, required by Ohio law and/or promulgated under law, will begin the formation of your child’s Marlington Local Schools permanent file. No application will be accepted until all copies of all necessary
and current forms and documents have been provided. Marlington Local Schools reserves the right
to postpone enrolling a student until such time as all documentation, forms and
information provided by the custodial parent/guardian have been verified as
true and accurate.
NOT PROVIDED for open enrollment students.
If your child is interested in participating in sports at
the middle or high school level, you must contact the athletic director for
information about open enrollment and athletic participation. All student athletic participation is
governed by rules established by the Ohio High School Athletic Association
(OHSAA). Mr. Steve Miller is the
