  • Custody Documentation (if applicable)
  • Proof of Residency (See list of documents listed in “Student Enrollment Guidelines”)

Please review the following information, complete the forms and bring with you to your registration appointment (all students):
Immunization Summary for School Attendance (provided by Ohio Department of Health)
All students will receive the guidelines and application form for free/reduced price lunches. Parents are urged to complete and return the application form as soon as possible. Questions regarding free or reduced price lunches should be directed to the Food Services Director at 330-821-4017.
The following forms will be completed on-line prior to the first day of school. After registration you will be supplied with instructions and log-in information to complete.
Media/Directory Release Form
Throughout the school year we receive numerous requests from area media (newspaper, radio, television, etc) for photographs, slides, audio and/or video recordings that feature students’ projects and achievements in academic and extracurricular programs. This form will give parents and opportunity to grant or deny the school permission to include their child in such media activities.
Emergency Medical Authorization Form
State law requires school district to have an emergency medical authorization form on file for each student. This form is required to be updated each school year.

Pay-For-It Services
Parents or guardians of Marlington students may go to the “Pay For It” link on our website www.marlingtonlocal.org or visit www.payforit.net. This allows you to create a secure online account where you can add money to your child's cafeteria account or pay school fees with your credit card, debit card or even an electronic check. By using Pay-For-It to put money on your child’s cafeteria account, parents can have peace of mind knowing that students will have money available for meals, and that the money will only be used for breakfast or lunch instead of other items. Please be aware that it can take up to four hours to post into the students account. Additionally, parents are able to view what the student has purchased, sign up for reoccurring payments and/or email alerts on low balances, balance reminders and upcoming payments.
Internet Convenience Fee
An Internet Convenience Fee (ICF) is charged for each transaction and is displayed separately when you process a transaction. Beginning June 1, 2013 the Internet Convenience Fee will be reduced to 3.9 percent
            4 Easy Steps to Get Started:
  1. Register: A parent or guardian completes the online form that includes their credit card information. An email          with a confirmation code is then sent to the address that was entered during the registration process.
  2. Validate: The parent or guardian enters the confirmation code, which activates their account.
  3. Add Students: Once registered the parent or guardian can begin to add students to their account and begin funding their meals or paying school fees. To accomplish this, you must know your child's student 9 digit ID number (which you should find on their report card or schedule), School building and Grade. This is a requirement for registering your student to the account. If you do not know their Student ID Number (Not their PIN) you can call the school and they will provide it to you or look on their report card.
  4. Deposit: Using the Make Payment page, you can now deposit money into any or all of your student's accounts. Money deposited before midnight will typically be posted to the students account by the following morning.
