All interested parent(s)/ guardian(s) must complete an Application form. Only a parent or guardian who has legal custody may complete an Open Enrollment Application. You will need to provide court documentation to show legal custody if there has been a divorce or separation. Application forms can be submitted from April 14th and ending on May 14th.

All applications will be dated and time-stamped upon receipt. Your application will NOT be marked complete until you hand-in the following supporting documents to Mrs. DeAngelis after completing the application: TWO proofs of residency (MUST HAVE a current date, name, and address), parent/guardian drivers license, IEP/ 504 (if applicable), immunization record, birth certificate, court custody papers (if applicable), and social security card (optional).  Failure to provide these documents by the May 14th deadline will result in a DENIED application.

Decision letters will be mailed no later than June 13th, 2025.
